Further delivered with a Patek Philippe Certificate of Origin dated July 1, 1996, wooden presentation box, setting pin, Patek Philippe platinum hard case back, leather envelope and outer packaging.

Manufacture of reference 5004 was launched in 1995. The model is illustrated in Patek Philippe Wristwatches by Martin Huber & Alan Banbery, second edition, p. 312.
